Use this form only for sending required information for child’s Deed Poll
Please check all spelling is correct on the form before clicking on the ‘Submit‘ button at the bottom of the form. Spelling mistakes cannot be corrected as the data which you have supplied, together with the deed poll will be destroyed after you have confirmed to me that the details on it are correct.
Once you have received the digital PDF of the Deed Poll, please save it to your computer – and also make a backup copy on storage media away from the computer.
I would recommend printing out the Deed Poll on a good quality vellum or parchment paper and that you print out at least 5 copies for your witnesses to sign as some people will request to see an original and not a photocopy.
- All parties (both of the parents or guardians) need to sign this form unless only one person has sole parental responsibility. If one party disagrees then a court order is needed in order to change the child’s name. Please seek the advice of a solicitor if this is the case.
- Non-binary identities are not legally recognised under UK law at present (hopefully this will change soon). However, after obtaining legal advice, I have created a Deed Poll that she thinks would be legally acceptable, so I am prepared to offer this option should you wish to use it. Please note though I cannot promise that it would not be challenged, this is how case law is established in this country, so it would be supplied to you on that basis – untested in a court of law.
- The data which you have supplied together with the PDF of the Deed Poll will be destroyed after you have checked the accuracy – so please check the spelling thoroughly to ensure it is correct.
Deed polls cannot be backdated so please set either today's date or a future date.
I regret that any data submitted that has a date from the past will be rejected.
